qoLyf

MDR, posting, and YAKAP live on PhilHealth.

This door explains where those records sit. It never rewrites the contribution figures on Health · PhilHealth, and it does not walk through a login.

Member Data Record

Your MDR is the record of who you are to PhilHealth. Look it up on the public site — qoLyf does not hold a copy.

Posting

Your employer is responsible for remitting the premium. You can check posting on the PhilHealth Member Portal or the eGovPH app (PhilHealth Advisory PA2026-0042). This room does not invent a posting, and nothing on Health changes when you open help.

YAKAP registration

Register with, or change, a YAKAP clinic for yourself and your dependents through eGovPH or the PhilHealth Member Portal. A PhilHealth Identification Number (PIN) is needed (YAKAP FAQs, as of 31 Jul 2025). Registration is how outpatient care and the medicines benefit become usable. It happens on PhilHealth, not here, and qoLyf never asks for your PIN.

Dependents

Adding or updating dependents is done with PhilHealth: through the Member Portal or a PhilHealth office, with the documents PhilHealth asks for. Planning for your family in qoLyf does not enroll anyone.

Using PhilHealth for a hospital stay

The current steps for an inpatient claim — your member data record and the forms the hospital needs — come from PhilHealth. Ask the hospital’s PhilHealth desk or the Corporate Action Center, (02) 8662-2588 or actioncenter@philhealth.gov.ph. qoLyf does not file claims and does not invent a claims process.

Official starting points

Benefits, accredited providers, and the Insurance Commission’s rules for HMOs and insurers. They are starting points for your own checking, not a certification of any plan. Nothing here sells a product.
